HOSPITAL LINK WORKER
Salary: £8,043 per annum (dependent on age & experience)
Hours: 16 hours per week
Mon -Fri with some evenings/weekends according to service need
Duration: 1 year contract initially

The Hospital Brain Injury Link Worker will work with Hospitals and other BI organisations to develop visiting / clinic opportunities on the appropriate hospital wards within the geographic area of Birmingham and Solihull. They will support and signpost brain injured people and their carers / families to the appropriate services for their needs.
Headway Birmingham & Solihull requires a reliable, trustworthy, self assured & self motivated person who has a good appreciation of the local framework of health and social services provision, with experience of dealing with people in traumatic and emotional situations and an understanding of the consequences of brain injury
To provide an assessment of the family and the brain injury needs, thus helping the families and brain injured people to move forward with their lives again.
Ensure that information about Headway’s services is communicated effectively to the brain injured people, their families / carers being treated at local hospitals or otherwise becoming known to us
The successful candidate's Key Responsibilities:

Provide emotional support as well as advice and guidance for brain injured people and their families / carers

Provide Level 1, 2 & 3 support and refer to HBS support services and Carers events

Work closely with Headway Birmingham & Solihull Case Workers.

Liaise with hospital staff / social services over hospital discharge and subsequent care plans

Develop relationships and networks to reach as many people as possible at the early stages of brain injury / caring

Visit Brain Injured people in their home, in hospital and at assessment centre visits

Travel across all areas of Birmingham and Solihull as required.Use of own vehicle, full driving licence is a prerequisite of this role.

Produce all required personal monitoring items accurately and on time.i.e., end of month, expenses, outputs and timesheets
This role is based in Rednal, Birmingham, but will require the post holder to travel / work at other Headway buildings in Moseley & North of Birmingham as well as travelling across Solihull. A current driving licence is essential, as is ability to use own transport for work purposes
